This for can be used to register a newly installed system or to register a replacement pump on an existing system. If you are registering a replacement pump, you will need to include both the serial number of the pump and of the unit in which the pump was installed.

Single phase versus three phase pumps:

When a single phase pump is installed, only one amperage field is displayed for each pump. If a three phase motor is installed, you will need to fill out amperage readings for all three lines.
